Registration Forms
Every high school student must complete the registration packet each school year. During registration families will complete the required back-to-school paperwork including:
Student Demographic Form
Mckinney-Vento Form
Student Handbook Addendum
Technology Agreement
Student Handbook Acknowledgment
Please note: Registration is a joint responsibility between the parent/guardian and the student. While we know many parents are eager to take care of the paperwork, we encourage students to actively participate in completing and reviewing their registration forms. This helps students become familiar with important school policies and expectations while taking ownership of their education and class schedule. Both the parent/guardian and student are required to sign the Technology Agreement and Student Handbook Form.
Arizona Residency Verification
Please bring one proof of Arizona residency, such as:
Arizona driver's license with your current physical address
Utility Bill
Pay Stub
Vehicle Registration
